Is the Vampiress going to be one of the 5 metal characters you get if you pledged $225 or more, or is she not?
Also, I want to make sure I do the special extras section of the survey correctly.
So for items that I want multiples of, how exactly do I fill that out?
Like if I want 2 werewolf sets, do I fill it out as Werewolves(2) or Werewolves, Werewolves?

I'd put it as:
Two units of 3 werewolves (6 models) with BOGOF for a total of 12 models ($50)

They replied to me and said most were writing it as 2 x werewolves, so that is what I decided to do as well.
I just wrote "BOGOF Werewolves", "BOGOF Trolls" etc

I put unit numbers and dollar amounts just to be sure. Like this:
1 Celestian Angel Paladins ($25)
2 Celestian Men at Arms ($50)
I also put at the top that I had $200 in credit. That way they could see what I paid, and make sure everything added up. Still I'm paranoid, and would have preferred some sort of form. (which I think would have saved them a lot of headaches)
I also wish they had sent out confirmation emails or something, with a copy of what you requested.
The special extras part was really unorganized. I thought there would be check boxes and a place to put if you wanted more than one of each, similar to the rest of the survey. Instead it was just like a comments box so that every person might interpret it differently what they should put in the box.
Its a simple choice, you can spend ages writing software to do a form and interpret the results or you can put in a text box and let the humans sort it out. If anything is not clear on your order then Mantic will be in contact to sort it out.

How can it take ages, when they already had choices exactly similar to what I expected for the BOGO extras already in the form above it?
There was already multiple choice selections for if you wanted Ogres with hand weapons or guns, for if you wanted the elf or TK cavalry for pledges between $175-225 and asking if you wanted your book early or would be ok waiting till august to receive it.
If they could do all that without taking ages to write up the software, then they could put down a list of all the special extras, where you could check off what you wanted and leave a blank space for if you wanted more than 1 of any of the options.
I realised I had forgotten to type in my name for display in the rulebook, and sent a message using the link in the survey mail. I received a reply within a day that they'd put it in manually for me.
So I have every confidence in Mantic for the handling of the survey. It's very true that it seems highly chaotic with no uniform input method, but it's clear that they're going over the filled-out forms individually, so if anything is unclear they willl contact you. So as long as you are uniform with your input (ie. don't mix it up like: 1x Werevolves, Men-at-Arms (30), 6 Trolls), I don't see any problems. The workload must be enormous for them to do it that way, but I have abolutely no doubt as to the quality of that work.

It's not too late. Contact them through the Kickstarter page and they'll get you sorted.
They didn't make the Special Extras section multi-choice or anything else because the survey software didn't allow for the complexity.

I like it, though. Mantic goblins are some of my favorites in terms of design - still annoyed at their hybrid status, but at least the price is better now *nod*
